Anus pain and straining is a major health problem that many people are reluctant to talk about but severely affects their daily lives. Its main symptoms are severe stabbing pain during bowel movements, bleeding, and discomfort when sitting. The main factors leading to this are changed diet and constipation. Anal Fissure: The most important scientific reason behind severe anal pain is anal fissure. When you have to strain excessively to pass hard stools due to chronic constipation, small tears or scratches form in the soft skin (mucosa) of the anus. This is a fissure. It causes severe pain and slight bleeding that lasts for hours during and after bowel movements.
Piles / Hemorrhoids: This is a condition in which the blood vessels around the anus become swollen and thickened. Piles form when there is excessive pressure on these blood vessels due to pregnancy, obesity, or sitting on the toilet for a long time without defecating. While internal piles do not cause pain, external hemorrhoids that occur outside the anus can cause severe pain and discomfort while sitting. When blood clots form in these blood vessels (thrombosed piles), the pain can become unbearable.
Anal Fistula: An abscess is a condition in which a small tunnel or tube forms from the inside of the anus to the outside skin to drain this pus. This causes chronic severe pain, swelling, and pus and blood to drain from around the anus.
Perianal Abscess: This is a condition in which the tissues around the anus become infected with bacteria and fill with pus. It looks like a large abscess in the anal area. Its main clinical symptoms are chronic severe stabbing pain while sitting and walking and high fever.
Bowel Infections and Other Causes (IBD / Proctitis): People with chronic ulcerative colitis and Crohn’s disease (IBD – Inflammatory Bowel Disease), which affect the intestines, often have persistent swelling and sores in the anal area. Along with this, infections of the skin around the anus (Proctitis), and frequent washing of the anus due to severe diarrhea can also cause pain.
